Since I got FireFox 15, I can not figure out how to make it open all web sites as new windows, not tabs. When it does open sites in tabs, it closes all tabs without warning me as it is supposed to sometimes.

Maybe set the browser.tabs.opentabfor.middleclick pref to false on the about:config page and middle-click the links.

You can also hold down the Shift key and left-click a link to open the link in a new window.

Some links that specify a target window can be opened in a new window instead of a tab.
