How do I remove a duplicate synced device from my sync account?

I have three devices in my Sync account: my desktop, my laptop, and my phone. For some reason, the Sync on my home computer stopped working. I've had some corruption in my Firefox program files due to constant crashes, so I just reinstalled Firefox and it started syncing again.

The problem is, now on my phone I have three sync devices excluding the phone listed:

Firefox Home. Last synced: Feb 22, 2016 Firefox Work. Last synced: 14 minutes ago X's Firefox on Y (User/Computer). Last Synced: 11 hours ago

The problem is, the name of my "Firefox Home" profile is still "Firefox Home", not "X's Firefox on Y". I've read that devices not synced for more than 3 weeks are automatically pruned. How do I get rid of this ghost device?

Did you try to disconnect Sync and reconnect Sync?

If that still doesn't work then remove the old Firefox Sync account and create a new Sync account.
