I'm using Firefox 3.6.13 on a Windows XP machine.

In View/Toolbars, the "Menu Bar", "Navigation Bar", and "Bookmarks Bar" menu options are missing.

There is a horizontal line and the "Customize" option below it.

I uninstalled Firefox and reinstalled it with no effect.

Problem solved. It was a particular button in the Custom Buttons addon.
