You may have an unwanted add-on or some malware installed. According to one post I read, this can be caused by one that has the following description:

"Flash Loader initial.rev64 by christiandavid (This add-on allows your browser to play online Flash content. Last Updated May 22, 2012)" (source: AVAST forum: Unknown url keeps popping up and getting blocked....)

To root out any suspicious or unwanted add-ons, try here:

orange Firefox button or classic Tools menu > Add-ons

On the left side, click the Extensions category and Disable everything that is non-essential. If you find the above add-on, remove it.

On the left side, click the Plugins category and Disable everything that is non-essential. If you find the above add-on, remove it.

Then restart Firefox and see whether the problem is gone.
