I have a window open with 3 tabs, I start working in one of the tabs and the tab I am working in opens in a NEW window, and a second tab opens with it. It only does it occasionally, but it annoys me that I can't figure out why?

Current Firefox versions have a feature called tear-off tabs.
You can detach a tab from the current window and open it in a new window by dragging a tab in the browser window.
You can drag that tab back to the tab bar in the original window to undo that detaching.
