I have version 50.1 and recently reformatted my hard drive and reinstalled windows. I of course saved my bookmarks and reloaded them. HOWEVER, the icons for all my bookmarks are gone - cant refresh, tried deleting and re-bookmarking, there is an add-on but only works to version 48 .... Really ??? something so simple is made so difficult or impossible ...

What file type did you save and reload?

The bookmarksbackup files in JSON or JSONLZ4 format don't contain the Favicons for bookmarks. The HTML backups and the places.sqlite file do contain those website images.

Whichever, those images will get saved as each bookmark is used the first time. As far as the older extensions that "grabbed" those images, apparently the extensions that were available in the past have been abandoned and no one has "adopted" them & updated them in quite awhile.

If you click a bookmark to revisit a link then Firefox should retrieve the favicon.

Do favicons otherwise work, like in the history and on the tab bar?

You can check the browser.chrome.* prefs on the about:config page. You can reset user set (bold) pref(s) on the about:config page via the right-click context menu to the default value.

You can open the about:config page via the location/address bar. You can accept the warning and click "I'll be careful" to continue.
