First click on a web page does work. And first click after switching from another application. After the first click, the cursor does not change to 'live' symbol (hand) when hovering over a link, nor show tool tips , nor open/close tabs. Can't click on an item from a drop down list. I've restarted Firefox, restarted the computer, and the problem is the same.

Do you have that problem when running in the Firefox SafeMode? http://support.mozilla.com/en-US/kb/Safe+Mode

Don't select anything right now, just use "Continue in SafeMode."

The problem is the same in Firefox SafeMode

Make sure that you have the latest version of the mouse driver software.
Also make sure that all mouse settings are the default (at least for the buttons and for scrolling).
