how can I request the browser to ALWAYS ask to remember username and password for a site? I did not use the remember function for a site once and then no more.
I want mozilla to always ask me when a password change takes place or use new sites (username and password). mozilla stopped asking me to remember changes to personal credentials after I missed to click remember a couple of times for this particular site: see attachment. One unusual feature this bank-site offers is that you can generate and use your own username that is associated with the one their system generated.

Hi, have a look at If you later change your mind and would like Firefox to ask you to save usernames and passwords for this site, you'll need to go into your Firefox Options and remove the site's entry from the Security panel exceptions list. in this article: Password Manager - Remember, delete and edit logins and passwords in Firefox. If you are never being asked for any site, make sure Remember logins for sites in the Security panel is selected - see linked article.

The site is not and never was on the Exemption List. I have no sites in the Exemption List.
If you are having problems with a specific site, try deleting ALL references to the website in question, as shown under the heading 'How do I remove a single website from my history?' in this article - Delete browsing, search and download history on Firefox. Note: It may take a few minutes to complete the deletion.
If you want to remove a saved user name; select it, then press the Delete key: Control whether Firefox automatically fills in forms
Also make sure you do not run Firefox in Private Browsing mode (never remember history).
