Do you have the McAfee Site Advisor, that one has caused this problem for some people. If you have it you can either disable or uninstall it to stop this from happening.

If you don't have the McAfee Site Advisor, there will probably be another add-on causing this. Use the procedure in the following link to identify what one - https://support.mozilla.com/kb/Troubleshooting+extensions+and+themes
