Is your account POP. I assume it is, do you have an antivirus program scanning your incoming mail? If so turn off the email scanning. the scanner dropping the ball is the usual cause of duplicates of existing mail in a POP account. Having the scanner scanning the Thunderbird profile is the next most common cause. The recent downloads do not get added to the popsate.dat file because the AV is scanning it. So next time they get downloaded again.

I have a feeling your issue is the second as you suggest it is intermittent.
