Internally, if you look at a contact's details, you'll see that the address book offers separate first and last name fields.

If you export an address book to a csv file, then you also see separate first name and last name fields.

So, i don't think you have a problem. You need to discover the two keys you need to use in your mail merge to access these discrete parts of a contact's name.
