This happened when Firefox updated itself to Ver 57.0 64-bit. Web sites run fine but no audio icon appears in the upper site tab and there is no sound. I did all the standard stuff (mixer controls, reinstall Realtek Hi Def audio drivers, run is Safe mode, etc. All other applications on the system are fine and have audio, including other browsers.

uninstall Firefox. Then Delete the Mozilla Firefox Folders in C:\Program Files and C:\Program Files(x86) Then restart system. Then run Windows Disk Cleanup. Then run it again and click the button that says Cleanup System Files. Note: your Firefox Profile is saved. But you should make a back up before you do : https://support.mozilla.org/en-US/kb/back-and-restore-information-firefox-profiles

Reinstall with Current Release Firefox 57.0 with a Full Version Installer https://www.mozilla.org/firefox/all/

Please let us know if this solved your issue or if need further assistance.

Nope; didn't work. Went thru the whole process including a clean boot after the uninstall just in case. It's the strangest thing; the browser seems unaware that there is audio originating from the page. The little speaker icon never appears in the tab as it did in the previous version, yet everything runs fine as if there wasn't a thing wrong. Sigh.

Thanks for all the ideas everyone, but it was getting to be too tedious so I did what I thought best: downgraded back to Ver 56 and sure enough, audio came back just fine. I'll turned off the auto-update and I'll wait until 57 has it's bugs cleaned out.
