I start Firefox in normal mode and open private windows as needed. I never get the warning for any private window -- it just closes immediately. I believe this has been the same pretty much forever.

If that is the only window open in Firefox at all, I wonder whether you are getting the warning about closing the browser instead of about just closing the tabs in one window?
I do have an extension which is a bit primitive but might be useful if you have private windows you can't afford to close accidentally. In that window, click the extension's toolbar button to open a special tab and activate the protection in that tab. More info on the Add-ons site:

I tested the old firefox 56.0.2, the warning works fine in all situations.
