You also could check Firefox's Error Console (Ctrl+Shift+j). For best results, click the Clear button at the top, then reload the page and check for Errors. Use the Errors button at the top to filter out the style warnings and other less relevant messages. Then try the image upload/insert and check for any new errors. Although you usually can't fix the errors yourself, they may provide some clues.
