Downloading any files from any web site using Firefox: first I see a Firefox download window with a progress bar and the word "Canceled" (Why? By whom? I did not cancel anything...) If I double-click that progress bar, a download begins. I can see two icons in a destination folder (one zero-length and one partial download) and the progress bar moves, everything seems to be right -- until the end of download, when the new file just disappears, annihilates, vaporizes from the computer! What is going on?

I encountered this problem on one of my servers running NOD32 Antivirus and FF4. Disabling virus scanning after downloads rectified the problem.

Steps to disable virus scan after download: http://support.mozilla.com/en-US/kb/Unable%20to%20download%20or%20save%20files#w_disable-virus-scanning-in-firefox-preferences

If this does end up fixing the problem for you it would be good to continue to follow this in the future so that if a better fix is found you can go back and re-enable this - it is good to have on.

While disabling virus scan after download fixed the symptom, it did nothing to resolve the problem and in fact may have made my pc more vulnerable. Why did this start happening to begin with? What virus scanning engine is being invoked? One native to FF? Or the one I have running on my PC? Interestingly, the same problem occurred with IE9 at probably the same time. Chrome was unaffected. Virus scans (AVG), anti-malware scans (Malwarebytes, Spybot) turned up nothing. Could possibly have been AVGs latest release? Not sure. It's not easy to disable AVG to test. Better to uninstall it, test and re-install.
