Any help would be appreciated. Firefox users are converting at about 1/3 the % of our users on IE, chrome and safari. So I'm wondering if something is happening to our form when firefox users are on our website - maybe because of add ons, etc?

I did get an initial rejection on a phone number I entered, but a different fake number was fine. The initial rejection cleared the four drop-downs at the bottom, which is a bit annoying.

You might test the page using Adblock Plus, a very popular extension which sometimes blocks page content mistakenly. I don't use Adblock Plus myself.
