You need to check to be sure it's a Firefox addon icon from the Firefox Addon site to see it exist there.

Not sure how to do that. When I look at add-ons extensions from the menu I don't see any way to add and icon.

You need to use the new extensions button. Your extension should be listed on it. Right-click it or click the cog next to it and select Pin to Toolbar.
