I've had this happen on multiple login forms, where the username field has the word 'password' in it, and the password field has some hashes. I've tried it out in other browsers and only Firefox has the problem.

Sounds like you may have a master password set. You can turn that off in the password settings.

That may be prefilled by the website or Firefox is showing placeholder. Firefox 4 supports showing (grayed) placeholder text for input forms as a hint that previous versions do not support.

No master password is set. Though it is set to remember passwords, but it's happened on sites that I've never been to before.
