I installed the Fx4 theme, and removed both the menu bar and the App button. As a result, I can't seem to find any way to access the File, View, or Tools menus. I googled for a list of keyboard shortcuts, but there doesn't seem to be one for any of these options (and no, pressing Alt does not unhide the menu bar, as it does in some other things).

Do I need to reinstall?

If you can't see your menu bars and pressing Alt doesn't work, you might want to try to start up Firefox in Diagnose Firefox issues using Troubleshoot Mode and reset your tools and controls. See: Restore the Menu bar in Firefox

Unfortunately, that doesn't work. I can't get into safe mode -- there's no Mozilla Firefox folder in All Programs, and running "firefox -safe-mode" from the Start Search just starts up regular Firefox.

I'm running Windows 7 Home Premium, 32-bit, using Firefox 3.6.4.

You can only start Firefox in Safe mode if all firefox.exe processes are ended. See http://kb.mozillazine.org/Kill_application
