Most Visited Websites
I have a webpage displayed in my most visited sites that I cannot dismiss nor delete from history. I have tried dismiss about 10 times and delete from history at least 6 times. How do I get rid of it?

You can remove all data stored in Firefox for a specific domain via "Forget About This Site" in the right-click context menu of an history entry ("History -> Show All History" or "View -> Sidebar -> History").
Using "Forget About This Site" will remove all data stored in Firefox for this domain like history and cookies and passwords and exceptions and cache, so be cautious. If you have a password or other data for that domain that you do not want to lose then make sure to backup this data or make a note.
You can't recover from this 'forget' unless you have a backup of involved files.
If you revisit a 'forgotten' website then data for that website will be saved once again.
This can be caused by a problem with the places.sqlite and favicons.sqlite databases in the Firefox profile folder.
- use "Verify Integrity" button on the "Help -> Troubleshooting Information" (about:support) page
If errors are reported with "Verify Integrity", close and restart Firefox or reboot and retry.
If "Verify Integrity" cannot repair places.sqlite, rename/remove all places.sqlite and favicons.sqlite files in the Firefox profile folder with Firefox closed. Firefox will rebuild places.sqlite and restore the bookmarks from a recent JSON backup in the bookmarkbackups folder.
- keep a backup copy of places.sqlite in case a new places.sqlite database has to be created
You lose the history when a new places.sqlite is created.

Does not explain why the Dismiss and Delete from history on the icon options do not work. Sometimes they work 1st time, sometimes they just move the icon, sometimes after a number of dismisses/delete/moving the icon all over they do finally work and sometimes no matter what you do they just move the icon around or do nothing at all. Why all the in consistencies?
