We have ESR deployed through SCCM with configuration set by GPO. Is there any way to make Firefox update itself without the end user starting the browser first? I'm thinking maybe a scheduled task that runs updater.exe or Mozilla Maintenance Service with some options or something similar.

Another option might be a scheduled task that starts Firefox in headless mode once a week, but I don't know if this will trigger an update.

You could use application supersedence in SCCM to push out updates. It wouldn't use the builtin Firefox updater, but it probably has the behavior that you want. It would likely work best if you disable automatic updates on Firefox.
