With Yahoo (and other sites) suddenly every single time I am required to use the "authenticator app" routine. I've gone for months or even years with persistent sign-ins before. What happened? How do I get back?

Fixed it. Somehow, "Delete cookies and site data when Firefox is closed" got clicked on.

Case closed.
