I keep getting cookie acceptance notifications even for sites I have already accepted

I keep getting pop-ups telling me a site wants to set a cookie. I have "accept cookies from sites" checked but the warning comes up anyway, even for sites where I checked "apply to all cookies from this site" many times in the past. I cannot find any way to make this stop. It is very irritating when I get a warning that says "this site already had 12 cookies" and the apply to all box is already checked and still it is asking again. What gives? BTW, I do not have "accept 3rd party cookies" checked, but still I have not asked for these pop-ups either. Please make it stop.
