Can't get address bar with Firefox button - either one or the other
With the toolbar options configured with menu bar, nagigation bar, bookmarks bar and tabs on top I get no Firefox button, a full menu layout before the address box. If I uncheck the menu bar, I get the firefox button, but no address bar, and a blank line between tabs and bookmark bar
Unchecking the navigation bar removes the empty line between tabs and bookmarks.
I have removed all incompatible add-ons, and looked for any likely culprits in about:config without success. Any suggestions?

It sounds like you have added the address bar and other toolbar icons to the menu bar, and have a blank navigation toolbar.
Use the method in the following article to drag and drop the address bar and other icons back onto the Navigation toolbar - https://support.mozilla.com/kb/How+to+customize+the+toolbar
Alternatively, use the method to "Reset toolbars and controls" as shown in this link - https://support.mozilla.com/kb/Back+and+forward+or+other+toolbar+items+are+missing
