twice recently I have lost folders and all the bookmarks in them - I haven't a clue how
I have twice recently lost some (not all) bookmark folders and all the bookmarks in them while I have been browsing the internet (i.e. while actively using Firefox - NOT on startup or shutdown). I can restore them from my backup but this is VERY annoying and I haven't a clue how this is happening.

p.s. I haven't just accidentally moved them to another folder - when I search for them they aren't found.
Did you try to use "Organize > Undo" in the Bookmarks Manager (Library)?
Are you using Sync to sync the bookmarks?
If you use Sync then best is to disconnect Sync while performing maintenance/repair efforts to the places.sqlite database.
You can check for problems with the places.sqlite database (bookmarks and history) in the Firefox profile folder.

The Bookmarks Manager is called Show All Bookmarks in Firefox.
To open the Bookmarks Manager, press the Alt or F10 key bring up the tool bar, and select Bookmarks. Hot key is <Control>(Mac=<Command>) <Shift> B.
