Problems with Add-ons needing reinstallation & "extensions.sqlite."

Everytime I start up Firefox it directs me to allow add-ons that I've already installed to install again. I've narrowed the problem down to "extensions.sqlite" by deleting it and seeing that it being deleted causes the same problem, but I can't for the life of me find any help as to how to fix extensions.sqlite from messing up. Everytime I start up Firefox a new one is generated, but it just causes the same problem. I don't have the same problems running on Firefox 3.6 or 4.0. I also have it running on Firefox 9.0.

more options

print or save a file from about:support which would indicate the extensions you have and if they are enabled or not. But you probably have seen all of your extensions become enabled already.
