Do the window elements show up properly if you start Firefox in Safe Mode?

If so,
1. Disable hardware acceleration under Options - Advanced - General.

2. Type about:support into the address bar and press Enter. Copy the Graphics section to the clipboard and paste it in a reply here.
3. Is the problem now gone when starting Firefox normally?

To double-check, does

  1. Firefox display properly when started in Safe Mode?
  2. Firefox display properly in normal mode with hardware acceleration disabled?

Your graphics driver is very old. Check if an update is available.
