when I add bookmarks in the bookmarks toolbar are they supposed to show up in the menu as well?

When I add bookmarks to the bookmarks toolbar they do not show up in the menu and vice-versa when I add bookmarks to the bookmarks menu they do not show up in the bookmarks toolbar? I thought that bookmarks are supposed to show up in both places no matter which bookmark that I insert them under?

Thank you for that! Somewhere else I read that this is the way they work? I will have to be more careful when I add bookmarks so I don't go looking under the wrong bookmark for something. I am now thinking that I will keep most of my professional bookmarks under the menu toolbar and the more visited bookmarks on the bookmarks toolbar.

On the Bookmarks Menu, you can find all of the bookmarks you placed on the Bookmarks Toolbar under a single entry for the Bookmarks Toolbar. Usually this appears near the top of the menu.
(Note: I called up the menu using Alt+b; if you click the menu, the Bookmark All Tabs entry is not displayed.)

Again, I do not have duplicate folders ie..., On the bookmarks toolbar I have a folder called 1 Doctors on the bookmarks menu I have a folder called 5 Doctors they only show up as designated and not in each others bookmarks? I thought that the moment I put a folder and added some websites to it that it is supposed to show up in both bookmarks (bookmarks toolbar and bookmarks menu) and this is NOT happening. Using Alt+b did not reveal or hide anything either
Maybe Firefox cannot handle the amount of favorites that I have?

No, the Bookmarks Toolbar and the Bookmarks menu do NOT mirror one another. As noted in my earlier post, you will find the bookmarks you added to the Bookmarks Toolbar under "Bookmarks Toolbar" on the Bookmarks menu; they are not duplicated anywhere else on the Bookmarks menu.
Why do you want bookmarks to appear in multiple locations? Firefox is expressly designed to avoid duplication of bookmarks. If you want to create duplicates, you have to use roundabout methods (see FIrefox won't let me save the same website to multiple bookmark files).
By the way, Alt+b is simply one way to drop down the Bookmarks menu
