When I try to bookmark a site, and then try to make a second bookmark in another folder, the first bookmark disappears.
I often wish to have bookmarks to the same site in more than one folder. If I visit a site, then select Bookmark this Page, then assign it to a folder in my bookmarks menu, the bookmark succeeds. If I then repeat the action and choose another folder, the result is that I have a bookmark in the second folder, but that the bookmark no longer appears in the first folder.
My workaround is to bookmark the site once, then open Organize Bookmarks and copy the bookmark to as many folders as I like. That works, but is not the way I expect the product to work.

You can do a Copy & Paste in the Bookmarks Manager (Shift+Ctrl+B) to create a copy of a bookmark.
You can also drag the website's favicon on the left end of the location bar to a folder into the Bookmarks menu drop down list or to a folder in the side bar (Ctrl+B) or onto the Bookmarks Toolbar.
In the side bar you need to click to the left of the icon to prevent Firefox from opening the bookmark.
Unfortunately cor-el, while you have accurately described some of the ways that a bookmark can be created, and redescribed the workaround that I was already using, you have not addressed the problem. I should be able to make a bookmark in any folder using the menus or shortcut keys, regardless of whether or not I have previously bookmarked that page in another folder.
