I have to click/press backspace twice to go back, and this causes the forward button to grey out.

Whether by clicking the back button or backspace, I have to do it twice for it to actually go back one page. Pressing it once, assuming I'm at the most forward page, just ungrays the forward button.

After pressing it twice and the page goes back once, the forward button grays out. The history menu still shows that I've accessed the page I cannot forward to.

Right click the Back / Forward buttons to get a list of sites visited.

That's not the issue here. If I press back twice, Firefox forgets any pages that are 'forward.' If I right-click and use the list to go back, I get the same issue.

Try running Firefox in Diagnose Firefox issues using Troubleshoot Mode. If it functions properly in that configuration, then one of your add-ons is the culprit.

It remains that way in safe mode as well.

Please do the following.

  1. Click Help | Restart with Add-ons disabled.

  2. In the next menu, checkmark: "Reset toolbars and Controls".

  3. Click the button called "Make changes and restart".

If that doesn't work, you might have a locked or damaged places.sqlite file. See http://kb.mozillazine.org/Locked_or_damaged_places.sqlite
