tab close buttons disappear when URL launched from another app

When a new tab is opened via launching a URL from another application on Windows (e.g. Outlook), the tab close buttons disappear, even with only two tabs. By carefully grabbing the right window border and dragging *inwards* (i.e. making the window narrower), the buttons suddenly appear on all tabs, thus illustrating that it's not a problem with the tab widths.

Start Firefox in Diagnose Firefox issues using Troubleshoot Mode to check if one of the extensions is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).

Now I know about Safe Mode! The culprit turns out to be the add-on ColorfulTabs 4.8.1.6.2.1.
