Note that form auto-fill doesn't work in Private Browsing mode windows (Always use Private Browsing mode; Never Remember History).

I notice a Spigot Searchme 2.x extension in your System Details list that might cause Yahoo to be used as the search engine. There is also a disabled Amazon Shopping Assistant by Spigot, so you might want to look into this.
