I'm fairly sure that in 4.0 the orange Firefox button had a "Tools" item, but now it seems to have disappeared from the dropdown. Where did it go? I know how to access it via F10, but this should be necessary, should it?

Sorry, Tools was never in the Firefox button drop-down, but Add-ons and Options from the Tools menu are there.

I must have imagined it there.

Save customizing that menu (can you tell us how?), I suppose I'll just use toolbar icons.
