How do I get rid of "E-Centives Coupon Activator Netscape Plugin" in Firefox?

I have tried going to "tools/addons" and clicking on a remove link, but there isn't one for this addon. It just has "disable". I have tried searching Google for how to remove it and tried the suggestions listed. Nothing seems to get rid of this thing. It apparently was installed in '09. I realize disabling it will prevent it from running, but I want the thing gone from my computer.

Check if the problem is in Windows 'Add and Remove Programs' found in Control Panel and uninstall it from there.

Once you disable the add-on, restart and go into the plugin manager, You should be able to remove this add-on.

No. After doing that the only button available is "enable".

Looked there as well. Nothing. The only place that I can find that it shows up is in my Addons from "tools" menu. It may just be a listing or whatever with no real program behind it, but would it still be shown at all if that were the case?
