After installing a theme my minimize, maximize and close buttons appear as a solid-black box. How can I get them to appear again?

After installing and switching to a theme, the minimize, maximize, and close buttons at the top right turn into a solid black box. The buttons still perform as expected if I click in the appropriate area of the box, but the buttons are blacked out. If I restart in safe-mode, or switch the theme back to default, there is no issue and the buttons are back. I've tried this with several themes and they all have the same results.

Running Firefox 19 on Windows 8.

While writing this question, Firefox was downloading the latest update for 20.0, but after restart with the new version, the issue is still persistent.
