Menu Bar is displayed by default, and shows icons for other installed add-ons (NoScript, Adblock Plus), but there's no icon for the ISC Infocon Watcher. Add-Ons Manager page -> Extensions shows all of my installed add-ons, including Infocon Watcher, but it's the only one with no Options button. I've tried Remove/Re-install a couple times, to no avail. Problem has persisted through several recent upgrades of Firefox. Any reason why only this icon is not displaying, and/or suggestions on how to make it appear?

Hmm, I noticed an annotation on the "versions" page:

Known issues:
  • This version is incompatible with Firefox 40. A fix is pending.

https://addons.mozilla.org/firefox/addon/infocon-watcher/versions/

The problem is that Firefox 40 and later no longer use the "widget" module to add toolbar buttons but instead the "UI" module. I'm probably not a good enough hacker to fix the extension for you, but it sounds like they are aware of the problem and hopefully will issue an update. You could send them an email indicating your interest to urge them along: infocon.ibyte@xoxy.net
