is this a unified folder? Do you also subscribe to the Allmail folder? If you unsubscribe from all folders except inbox, do you still see multiple copies?

It is a unified folder. I don't have an Allmail folder.

Undoubtedly.

Go to the little square icon on the line to the right of the heading item account. Click it, and select location from the menu items offered.

Now repost a new image showing essentially the same thing but with the location column showing. That will remove any ambiguity as to where this first and second items is coming from.
