I can't get a new tab to open, either by pressing the + sign, going to file new tab or by right clicking and selecting new tab. What's the problem?

I can't get a new tab to open. I've tried pressing the + sign, going to file-new tab and by right clicking and selecting new tab. Why won't a new tab open?

Try "Ctrl+T" for a new tab.
