Firefox 4 Windows Missing New Tab icon in Custoimize Toolbar area
Firefox 4.0 Windows shipped with an "icon-is-not-there" display bug in it.
There is NO New Tab / Open New Tab icon IN the Firefox 4.0 Customize Toolbar dialog", at least in the Windows shipping code. In fact, there seems to be everything BUT that missing icon in the Dialog.
The programmers left the New Tab / Open New Tab icon out by error, thus, there's nothing to drag onto the toolbar for New Tab or Open a New Tab needs.
If your copy of Firefox 4.0 Windows DOES have the New Tab / Open New Tab icon IN the Firefox 4.0 Customize Toolbar dialog, then you are using a version of Firefox 4.0 shipping that no one else has.
The Mozilla programmers please to rectify the mistake of the missing New Tab icon at their earliest convenience.

You can drag the New Tab (+) button from the Tab bar on another toolbar to make it appear there.
If you do not see the New Tab button on the tab bar then click the Restore Default Set button.

On most all Firefox 4.0 installs, the + Open in a New Tab icon is NOT movable at all.
It can not be dragged anywhere, no matter what one does, which means it can not be placed in any active Toolbar..
The + being discussed here is the one that appears in-line with the Tab bar of open webpages just to the right of these tabs, in the upper right screen section, just to the right of the > symbol.
This is expressed mainly in the FF4 Windows OS version.

Firefox 4 technically has two New Tab buttons. One on the Tab bar that always stays there and a second New Tab button that can be dragged on other toolbars. If you drag the New Tab button from its default position (shows at the right end of the tab bar if Customize is open) then that button becomes a regular toolbar button and the default New Tab button gets hidden. If it is not on the Tab bar then the icon changes from a + to the toolbar version of that icon.
Open the Customize window via "View > Toolbars > Customize" or via "Firefox > Options > Toolbar Layout"

But the above message and description of how it is done is beyond belief ! -- yet it is NOT your fault for being so.
Who in the he** would ever think that this is the way to do it, in order to get a "New Tab" icon onto the toolbar ?
No one would ever think of that way to do it, much less realize that one would apparently first drag a "gives appearances of being stuck" "+" icon onto a DIFFERENT Customize area, THEN drag that icon that changes to "Open a new tab" BACK onto the toolbar so it can be used ?
I have it working now, but someone at Mozilla Programming is playing a joke on the users.

ok I used Cor-el's solution and agree with Joseph that the default implementation is horrible.
however, i'd like to have a new tab button in the regular toolbar and at the end of tabs, which is something i now can't seem to do. once i drag from the customize toolbar window, i can only do so once.
Just double click to the right of the last open tab and as if by magic a new blank tab appears.
My new tab icon comes and goes. Sometimes it appears to the left of the tabs , sometimes to the right, sometimes on the very right side of the tab bar. Most of the time it is just gone. It does not reappear in the customize toll box either, it just disappears into the bit bucket for a few days and will reappear for a short time when it wants to. I tried reinstalling firefox to no avail. It is a curse!
