How do I close Firefox help screen?

When I open Firefox, the help questionnaire pops up. I click on cancel and close. Then the Firefox help page comes up for me to ask a question. If I try to close it, the questionnaire pops up again. It is an endless cycle. I cannot get to my home page, bookmarks, or anything. How do I stop this?

Every time Firefox opened

"Clear the Cache": Tools > Options > Advanced > Network > Offline Storage (Cache): "Clear Now" "Remove the Cookies" from sites causing problems: Tools > Options > Privacy > Cookies: "Show Cookies"
