Updates always create another icon in the dock, and the Mac won't shut down

For the last few updates, Firefox has caused the same issues on all four of our Macs (Pac Pro, MacBook Air, MacBook Pro & Mac Mini). All of them are currently on the latest OS X (El Capitan 10.11.3). The update appears to install and work, but a second icon appears in the dock. On trying to quit Firefox, it won't quit and then (even worse) the Mac will not close down. 'Shutdown' and 'Restart' do nothing. Force quit shows both versions of Firefox, but neither can be 'force quitted'. The only way to shut down the Mac is to switch it off at the wall. On re-starting the Mac, Apple informs that it was not closed down properly. However, after all this, Firefox then does work properly. I think it is installed correctly in the Applications folder, and I have (on one Mac) completely refreshed Firefox. but this hasn't helped. Is this a known bug, or am I doing something wrong on all four Macs? Thanks, in advance, for your help.

Try installing Firefox from an installer from here: https://www.mozilla.org/en-US/firefox/all/ The update procedure might be corrupting something.

Thank you. As I'm in England, I downloaded 'English (British)'. I've just updated my MackBook Pro and MacBook Air, and both with 100% success. I'd previously tried an update using a general Firefox download website, but that didn't help. This page that you've suggested has 'cured' the problem. It hasn't fully cured the problem, of course, as Firefox should be able to update itself. However, it's certainly a way of avoiding problems, thank you. It will be interesting to see if anyone has any cure for Firefox updating itself.

I will mark this as solved as you do not have a problem at this time and have a working solution.

Marking it solved makes it easier for others to see the thread and the solution.

Others with the problem should click the box [I have this problem too]

Note Mozilla are looking at a known issue that may be related. See

Some related threads are here you may find it far easier to follow progress in those threads than by tying to following the technical bug.

P.S. Note there is a method of forcing the Mac to shutdown mattowski said

... If you attempt to restart your computer at this point it won't let you. And now you have requested a restart you can no longer open any other applications to resolve the issue. ...

However some bright spark has deduced that you can do the following (provided you hadn't already tried to restart)
Open the terminal window and perform the following commands.
sudo killall launchservicesd
sudo killall Dock
