To restore it I have to uninstall/reinstall Firefox or do a full registry restore. Can you tell my why this happens and how to fix it?

If you are using Windows, maybe your icon cache is too small. When I was using Windows, I had to make it bigger. The icon cache could also be corrupted. http://www.thewindowsclub.com/rebuild-the-icon-cache-windows

Having more than one version of a program may cause issues. You must remove the older programs. Then get the current full installer.

Flash: Grab the uninstaller from here: Uninstall Flash Player | Windows Uninstall Flash Player | Mac

Then reinstall the latest version.

I tried both your suggestions and neither worked. I now only have one version of flash. I uninstalled firefox and reinstalled it and the problem went away. However, it will be back and there must be a better way to solve the problem than uninstalling/reinstalling firefox every time.
