Notification window opens when facebook is opened. How do I get it to stop?

Every time I open Facebook a window opens in the top left that prompts me to turn off or keep on notifications for Facebook. Nothing I do stops it. I would like to stop this as it's rather annoying and enough to get me to use another browser.

Here is a screen shot of what is happening. Only does this for FB.
