What's happened to the + sign to add a new tab? I've customized Firefox to have tabs below the address bar (as they should be) but have just noticed that, once the window is full of tabs, the + to add another tab has disappeared. Is this because of my customization or has Firefox removed this function?

Sounds like the CSS code you use is outdated and needs an update. You can check this question.
