I Remove Cookies and add their senders to the Exceptions - Cookies list. However, the removed cookies instantly re-appear on the Cookies list. Examples: 2o7.net, adbrite.com. Cookies immediately reappear on Cookies list even when deleted while cable modem is disconnected. FF 3.6.13 on Mac OS 10.5.8.

Do you have the Targeted Advertising Cookie Opt-Out (TACO) extension? That one will automatically add permanent, generic, non personally identifiable cookies to the browser. Even if you delete one the add-on will replace it.
