When clicking a download link on a website, I get the save/cancel dialog, but then Firefox freezes while the file downloads, gets scanned by MSE, then finally allows me to click the save button. Firefox should not attempt to download the file until I click save.

hello bunder, this is intended behaviour & won't change (refer to bug #295410).

if you're experiencing delays by virus scanners you could try to toggle the browser.download.manager.scanWhenDone preference in about:config.

Changing that option won't help much. The lag occurs "before" the file is downloaded.

it might be worth a try nevertheless - when it's a small file it might have been already downloaded into the temp-folder in the background before the save dialog pops-up...
