It seems that after the last few upgrades/updates, my menu customizations disappear when I restart. I don't use the Firefox button (just another added step) and after the restart the Firefox button is not there but my menu icons are missing.

I'm sorry to read that you are having difficulties with Firefox.

What I understand is that your browser resets to the original menu view every-time you restart the browser. Is this correct?

Could it be that you are stuck in Safe Mode? Are your Add-ons activated when you start you browser?

No, I am not in safe mode, all enabled add-ons are activated. Let me clarify; the Firefox button is not activated and the menu bar is checked when I right click on the menu area, but the icons are missing except those produced by certain add-ons; Lastpass, adblock plus, skype and imtranslator.
