The unified button is on the left of the address bar (between the favicon and the web address).

All of the screenshots show it as a green/red button attached to the right of the address bar... how can I get this?

I am not looking to seperate the buttons. I tried several combnations through the "Customise" menu, and only one of these (Address bar followed by reload button followed by stop button) integrates the stop/reload with the address bar, but it appears on the wrong side.

I discovered that the Fission add-on was causing this problem for me. This add-on modifies the address bar to show the progress bar within it, Safari style. After disabling the add-on then the unified button is on the correct side of the address bar.
