How do I set a specific action for an attachment type?

When I go to Tools->Options->Attachments, I see an area with a header of "Content type" and "Action", but there is no obvious way to add an attachment type and action to the list.

Actions are added when you open a new type of attachment and follow the prompts to define the associated opening application, and check "Do this automatically..." in the prompt dialogue. In some cases the "Do this..." box is greyed out and the action is not added, and if that happens reply here for a workaround.
