Could you explain the steps to re-create the problem. I see in your screenshot you have a local file open (file:/// URL). Where is the custom protocol coming in?

Try going to the Options page, General panel, Applications section, and find your protocol there (probably it is listed) and change the action to "Always Ask".

Now when you try the link with the custom protocol, does Firefox offer a selection of programs to handle it?
